Parents, please bring ALL the required documentation to the Central Office at one time or send it in ONE email. Partial documents will not be accepted. You can drop off the documents Monday-Friday from 8:30 am-3:30 pm. Your child must be FULLY potty trained.

Documents needed:

-Birth Certificate

-Updated shot record

-Social Security Card

-Driver's License

-2 proofs of residency

Slots will be filled on a first-come, first-served basis.
